Please help me find the right program.?

I am a performing magician and balloon artist out of Pennsylvania usa. I am looking for a program that will allow me to construct a form and be able to type in it to fill it out. Sort of like a free alternative to "Filemaker pro" (Filemaker.com) or Formstack.Does anyone have any ideas? It would be greatly appreciated!!! :D
